Case Manager-Mental Health Court

We are seeking candidates for a Case Manager to provide supports and services for individuals involved in Mental Health Court. 

Essential Duties and Responsibilities:

  • Regularly  presents to approved and applicable jail(s) and meets with currently incarcerated persons interested in Mental Health Court to explain the concept of Mental Health Court
  • Assists in the completion of Mental Health Court applications and obtains multi-party release for Mental Health court so that the cases can be staffed. 
  • Processes clients for eligibility through screenings and coordination of documentation to determine Mental Health diagnosis and/or co-occurring concerns ensure that the potential clients meet the requirements of the program.  Screenings and paperwork may include Social history, Brief Jail Screening, GAINS, Criminal background information, etc.
  • Present potential client information to the Mental Health Court team with appropriate releases in place.
  • Assists in admission paperwork for Community and Family Resources/The Richmond Center to facilitate Diagnostic Assessment or Initial Assessment
  • Documents and tracks all clients who are and are not admitted to the program.
  • For all clients admitted to the program, meets with the client weekly and completes case manager goals. Assists the client in accessing resources which could include filling out payment source applications, public assistance needs, job applications, public transportation and/or any other resources needed by the client.
  • Participates in all Mental Health Court dates and staffing’s as scheduled and collaborates as needed with the mental health court team
  • Provides Phase applications to the client and assists as needed with the direction of the Court.
  • Actively works on community outreach on a regular basis
  • Acts as a liaison to and consults with community agencies, law enforcement agencies, jail personnel, County Attorney/Public Defender, Defense attorneys as appropriate.
  • On a quarterly or semi-annual basis assists the Mental Health Court team with a review of the Policy and Procedures and forms
  • Completes all required documentation in a timely manner
  • Other duties as assigned

Schedule:

  • Type: Full-time
  • Hours: Monday-Friday, 8a-4:30p

Education and Experience Qualifications:

  • Bachelor’s degree in a related field and at least one year of experience in a case management role is preferred; OR
  • Associates degree AND three years of related experience in a case management role may be accepted in lieu of a Bachelor’s degree.
  • A valid Driver’s license and proof of Automobile Insurance is required.
  • Must be able to pass the background check required by approved and applicable jails.
